I'm getting back into cake decorating after quitting for several years. The bride and her mother came by today with the wedding cake topper. It's a resin bride being carried by the groom and is very heavy. Is inserting wooden dowel rods in the top tier my only choice to support this very heavy topper?

In my opinion, doweling would be a must to keep it from sinking into the next tier.
I just dealt with this issue this weekend. I had a very heavy and big ceramic topper. I was very nervous about the topper sinking. I think your best bet would be to use at least 1-2 plastic white dowel rods. I ended up using 3 white ones(one in the middle and then one on each side)...I then used some wooden dowel rods as well. I did not have any problems with the topper sinking. You could use a cake board cut to size underneath the topper if you are going to have some kind of flowers or something else on the top tier that would conseal the cake board. If not, you will be fine just using the dowel rods.

I recently used that same topper and it is on the heavy side! I would use dowels and underneath the topper, I would put a cake board cut to the same shape. ...rather be safe than sorry!
If it's really heavy, I use pvc pipe to support it. You can get pvc pipe at any hardware store from 1/4" in diameter to 4" in diameter.
