How soon after putting on my sugar paste/fondant do I have to put on the extra bits. For example I'm making a cake with trains, clouds letters and stuff. Do I have to put them on as soon as I've put the fondant on, or can it set and be done the next day? Sorry very long winded for a simple question.

Smaller decorations can be put on the following day.  Just use some water or piping gel to stick the decorations to the fondant covered cake.

In a single word?  Forever :)  Once a cake is covered w/fondant whatever other needed can wait as long as you want.  As Freckles said, just a very light smear of water of piping gel will adhere decorations to the cake.
