I've done a similar cake the other day. Simply stick a skewer or something like that into the fish when you form it, let it dry and then push the fish with the skewer into the side of the cake.

I should say I actually used two, quite long ones and my cake was chilled.

I was just afraid of it being too heavy and tearing through the cake

Use RKT and then cover with whatever medium you wish...modeling chocolate, fondant, etc.  It won't be as heavy that way.

I would suggest using a mini doll skirt for the fish - probably adding the rice treats for the 'snout' part.  With a couple of straw/skewers  it would be fine

Every time I use RKT and then crumb coat it and put fondant over it it's almost lumpy

In this case I think the body being a bit lumpy would just add to the 'realness' of a fish :)

you have to REALLY mash the RKT until they are sooth (ish).

Like others said RKT will be your best bet for the fish. I use kelloggs one, they can be smoothed easily. Use skewers dipped in melted chocolate on both ends. That will ensure the firmness and hold. My two cents. Hope this helps :)
