A Different Pillowed Technique...how'd She Do It

Ive stared at this photo and cant fathom how she made pillow effect on top tier! Any thoughts?

Looks to me like pieces of fondant/gum paste were attached the crumb coated cake and then the fondant was laid over the whole thing to give that extra dimension.

Chris Garren's on Amazing wedding cakes uses this technique and that is how he does it ,and then details the insides with fondant tools to the effect he is looking for.
