Fondant?
3 tiers covered in red fondant with black butteflies and black satin ribbon around bases of tiers.
Silver Pearl Dust accents? Strand of small silver beads around base just below black ribbon?
Black butterflies on wires coming out top of cake? Black butterflies in wires coming out all sides of cake?

Buttercream...drawing a blank here....

White buttercream with red and black ribbon around base of tiers, red and black butteflies over the cake.

How about something like this: http://www.cakecentral.com/cake-photo_1781769.html

Or this, adding some white royal icing butterflies perched on flowers: http://www.cakecentral.com/cake-photo_1420977.html

Or simply black ribbons on the bases with a red swag draped down the tiers, with strategically placed white royal icing butterflies.

Then again, I thought this one is really cool, and adding some butterflies seems like it wouldn't be a huge deal. You could probably do butterflies in place of the flowers: http://www.cakecentral.com/cake-photo_1259025.html

I am a huge fan of the scroll work on the sides of the cake. I really like the one cake in the previous post.

Do they want a coloured fondant cake or cake with red and black decorations?
That would be a starting factor. As to butterflies -- use red and black with touches of white/cream/green. I could see them cut out of fondant/gumpaste mixture ( Cut out wings and attach to a body of royal icing) Let dry. Or you could do royal icing butterflies ( do designs over plastic/acetate) let dry be sure they are thick enough to survive. Scroll work or paisley designs would fit in well.
