How Do I Make A Toothpaste Tube?

It is actually a tube of surgical lube, I work in a GYN office and it's for a retirement party. I want it to be round, any ideas?? Thanks!

I would start with 4 or 6 inch rounds cut on half so it lays on it's side...Like 2D.Then carve the top of the tube and bottom area to the shape.
