Icing Overhang?

Every time I ice a cake I seem to get an overhang from smoothing the top of the cake. How or what can I do to prevent this?

When I smooth the top then smooth the sides I always end up with either the other hang or top mush over edge argghh!!!

Any tips on how to prevent this?
