Things To Ask For Cake Orders

I've been having fun decorating cakes for family and friends and on a whim, posted an ad on craigs list to see if there was any interest. Well I got a request! Of course, since this is a first potential paying customer, I'll be doing it for practically nothing, however, want to make sure I get all the info from the get go - what kinds of questions do you ask? Here is what I have so far:
I know its a communion
# of servings
Type of frosting
Type of cake
Type of filling


Am I missing anything? Is there a form that exacts out there?

Before any of this, make sure you are legal to sell cakes. But that's a whole other topic...

1. Servings
2. Shape
3. Flavor
4. Filling/icing type
5. Design/colors
6. Delivery location and time of event. Will cake be in or outside?
7. Phone, email, payment info (everyone prepays me, but that's up to you)
8. Alternate phone contact
