Shoe Box Cake Question

Please could anyone tell me or if there have been any former forums on this topic. How does one get the top of the cake box to look like its up and open and not crush the gumpaste or fondant tissue underneath it. Also is the top lid of the cake a cake on a rectangular cake plate or cardboard of some sort. I read some people use styrofoam covererd in fondant for the lid but I would like to use cake.
