My most recent fiasco happened 2 weeks ago, but thankfully I was able to fix the problem and still make delivery on time.
Due to an event taking place at the church, freezer and refrigerator space was at a minimum. So I left the bottom 2 tiers of a very small cake on the counter overnight while the cake top was in the freezer.
I put my center dowel into the two bottom tiers then began to slide the top tier onto the dowel, but (and I know better than this) in my zealousness to finish an event ahead of schedule...ha, ha, I didn't allow the top tier to defrost enough and it slid down until it go to the frosting center and stopped. Well my hands were melting the buttercream and it was all over my hands, so I went to grab a papertowel to clean off my hands so I could push the top center of the cake down. Well I thought the cake would balance at least for a little while....But alas no. I watched in horror as the top tier and the center dowel fell over tearing through the two bottom tiers! I cried for about 5 minutes, pulled myself together, repaired the cakes and re-iced & redecorated.
Thank goodness my interns were there to work on the food for the wedding. It did cause me to have to rush the grooms cake decorating so I wasn't happy with the quality of that cake, oh and did I mention that my fiance' was in the hospital, and had had his kidney removed the day before?! So I had a housefull of future inlaws from Virginia there as well. But I survived, and the bride LOVED EVERYTHING.
I have had other fiascos that didn't fare as well and gave discounts & free incentives to save reputation, but for the most part they are few and far between.
