Consultation Worksheet

I want to make up a worksheet to use during consultations. Nothing fancy just in Word so that I can make notes and make a copy for the bride to take with (or have once she books) so that we are on the same page about everything.
There have been pieces of info that I have missed or that brides have been unclear about and I want to avoid that in the future.

Does anyone have something like this that you are willing to share?
Any input for things to have on there would be appreciated.

So far I have:

date
time
cake/cupcakes
flavors/fillings
shape
colors
base price for bc/fondant
approx # of servings
location
add'l decorations-sugar flowers, butterflies, etc.-est price
photo?-book/mag title issue no.

That's what I've come up with so far. I welcome your thoughts on this. Thanks!

Reception date and time/Is in inside or outside
Number of guests
wedding colors, flowers, theme
Type of food that will be served...lunch or dinner
Cake shape
Cake flavor, filling flavor, BC, Fondant
Wedding cake ideas
Cake topper?
Grooms cake?

Contact person at the site; name, phone and email. (You may need to contact them to find out if they need a cert of liab insurance from you and/or a copy of your health license; plus it's good protocal to contact them to introduce yourself and find out best door to use, what special rules they have, etc., ... especially if it's your first time there.)

WHat time does the receptoin start? (This means "What time did you tell your guests to be there?") What is the earliest/latest you can deliver the cake?

Confirm type and size of table .. where will it be (so you don't set the cake up on the gift table or something). Confirm it will be covered with linens an ready to receive the cake when you arrive.
