I'm desperately looking for a picture of a blank patient chart. My best friend is an RN and I want to put a patient chart on the top of her cake with her name at the top as patient. Trouble is I cant find any images of blank patient charts online. (The only thing I see is computerized versions of such. I want the paper version). Anyone either know where online I could find one that I havent already looked, or if you are an RN or know one if it's legal or possible to make a copy of one? (the hospital/office information can be blocked out, I'll put in my own version)
