I made a gumpaste leaf arch topper for a wedding cake that would probably work for a wreath as well. Each leaf is wired with floral wire, then the stems are wrapped with brown floral tape, each stem laid end to end for length and then bent into an arch shape. You could draw a circle on paper for the size wreath you want to make, tape the stems as you go until they cover the circle you have drawn.

The grapes are gumpaste as well and the tendrils are just green floral wire spiraled tightly around a small paint brush handle, slip the wire off the handle, pull from both ends to lengthen it and cut to the size you want, then tape to the leaves with floral tape.
