since most estheticians focus on skincare and nails, you could have a women holding out her hands as if to get her nails done, or a women mid-facial...but maybe ask what the specialty is of the person the cake is for. There are many different procedures in this field, and many people have only certain ones that they perform. this could help lead you in the right path
